[openrisc] Or1k docs bug...

Trap exception is not listed in the bit field mask of
the Debug Stop Register (p. 324) or the Debug Reason
Register (p. 325). It is listed in the description fields.
I have added it at bit position 13 in my simulator.

Needless to say...the Debug Unit didn't respond well to
trap exceptions. :)

I am making steady progress on the debugger interface. I
will keep everyone informed.
